While we do make an attempt to accommodate patients with immediate problems and emergencies, all regular office visits are made on an appointment basis.  In the event that you need to reschedule your appointment, please notify us at least twenty-four hours in advance, so your scheduled time may be offered to another patient.

We look forward to meeting you at your first appointment.  In order to save time and make you first visit as pleasant as possible, please take a moment to download and print our "New Patient Registration Form".  Complete this form at your convenience and bring it with you on your first visit.  Be sure to bring along all other pertinent information such as insurance information and current medications.

Our billing department handles your insurance questions and claims submission, including workers compensation, and all surgical procedure claims.  We participate with many different health care plans and we accept Medicare assignment.  Patients not covered by these plans are requested to pay at the time of their visit.  For you convenience we accept cash, checks, Visa, Mastercard, and American Express credit cards.
