Below are a list of forms to use when referring a patient to our clinics.  Whenever possible, we ask that these forms be completed and sent to the clinic as soon as possible prior to the patient being seen.  Be sure to describe in as much detail as possible including the location of any defects seen.      Thank you!

If you need to reschedule an appointment, please notify us at least 24 hours in advance, so we can offer that time to another patient. If you have an immediate problem or an emergency, we will do our best to accommodate your needs.  If the problem is not an emergency and you do not require immediate attention, please call to make an appointment to see one of our doctors.

Do you have an eye emergency?

One of our physicians is always on call after office hours and on weekends to respond to an eye problem that requires urgent attention. Our regular office phones are answered 24 hours a day.  After office hours, our answering service will forward your message to the physician on call.
